Healthcare Union Urges Immediate Talks with Government Amidst Rising Challenges

The Government Medical Officers’ Association of Sri Lanka has urgently called for a swift discussion with President Ranil Wickremesinghe, Prime Minister Dinesh Gunawardena, and Health Minister Keheliya Rambukwella to address the serious challenges facing the country's public healthcare system and the pressing issues encountered by medical professionals.

In a special media statement signed by Secretary Dr. U.K.B. Warakagoda, the association emphasized that despite multiple requests for discussions over the past few months, there has been no adequate response from the government regarding these pressing matters.

The statement outlined the necessity of effective dialogue between the government and the medical community to overcome the current challenges facing the healthcare system.

Medical professionals play a pivotal role in ensuring the continuity of high-quality healthcare services while safeguarding Sri Lanka’s free health service. However, numerous issues related to the professional, economic, and service conditions of doctors urgently require attention. Timely and sustainable solutions to these problems are crucial for the future of healthcare in the nation.

While healthcare costs per individual have remained relatively low, Sri Lanka has managed to maintain health indicators comparable to those of developed countries. This achievement is primarily attributed to the dedication, expertise, and professional responsibility of healthcare workers, including doctors. To secure this victory for the future, it is essential to ensure favorable conditions for healthcare workers, fair service terms, and a sufficiently staffed, robust healthcare system.

The decline in the value of doctors’ salaries and allowances, issues surrounding tax burdens including the Advance Personal Income Tax (APIT) system, the loss of one out of three additional duty allowances, and the abolition of allowances for service vehicles have adversely affected the economic and professional status of medical practitioners. It is important to facilitate discussions among relevant parties to seek fair and practical solutions to these issues.

Additionally, the lack of a structured cadre revision process for medical staff since 2015, paired with the expansion of health services, rising patient needs, and the emigration of experienced doctors, underscores the necessity for a reevaluation of the challenges faced by the healthcare workforce.

Attention must also be directed toward ensuring that the salaries and allowances of doctors are aligned with their training, experience, and contributions to the healthcare system. Conducting a comparative analysis based on reliable data from countries with similar income levels could pave the way for establishing a specific salary structure for Sri Lankan medical services.

The proposals submitted by the Government Medical Officers’ Association for the 2027 budget form a critical part of the process of seeking solutions to these challenges. These proposals include restoring additional duty allowances and government leave days to their original state, amending the DAT allowance, introducing a risk allowance for healthcare workers, implementing fair tax reforms, creating a specific Sri Lankan medical service with a unique salary structure, and engaging in workforce planning and revision. The introduction of innovative services such as clinical travel medicine could also help generate foreign exchange.

Over the past few months, the Government Medical Officers’ Association has made several requests for meetings with the Health Minister, Prime Minister, and the President to discuss these challenges, highlighting the need for urgent dialogue on the matter.

The association urges the government to prioritize constructive discussions over accusations or divisiveness, working collaboratively with all parties involved to implement practical and sustainable solutions to the prevailing healthcare system challenges.
