මාධ්ය නිවේදනය
විශේෂඥ වෛද්ය සේවාව පවත්වාගෙන යාමට වගකීම් පමණක් නොව නිසි බලතල, නීතිමය ආරක්ෂාව සහ සාධාරණ සේවා කොන්දේසි ද අවශ්යයි.
රජයේ වෛද්ය නිලධාරීන්ගේ සංගමය අවධාරණය කරයි.
දේශීය පශ්චාත් උපාධි විශේෂඥ පුහුණුව සම්පූර්ණ කර විදේශීය පුහුණුව සඳහා බලා සිටින වෛද්යවරුන් දිවයිනේ විවිධ රෝහල් වෙත යෙදවීම සම්බන්ධයෙන් සෞඛ්ය අමාත්යාංශය මෑතකදී නිකුත් කරන ලද පත්වීම් වලදී ඔවුන් වෛද්ය නිලධාරීන් ලෙස හඳුන්වා තිබීම පිළිබඳව රජයේ වෛද්ය නිලධාරීන්ගේ සංගමය (GMOA) අවධානය යොමු කරයි. දුෂ්කර සහ පහසුකම් අඩු ප්රදේශවල ජනතාවට විශේෂඥ වෛද්ය සේවා ලබාදීම අත්යවශ්ය බව රජයේ වෛද්ය නිලධාරීන්ගේ සංගමය පිළිගනී. එහෙත් එම සේවාව පවත්වාගෙන යා යුත්තේ වෛද්යවරයාට පැවරෙන වගකීම් වලට ගැලපෙන නිසි පත්වීම, බලතල, නීතිමය ආරක්ෂාව සහ සේවා පහසුකම් සහිත ක්රමවේදයක් යටතේය.
විශේෂඥ මට්ටමේ වගකීම් සඳහා පැහැදිලි පත්වීමක් අවශ්යයි.
MD විභාගය සම්පූර්ණ කළ වෛද්යවරුන් ඉහළ මට්ටමේ පශ්චාත් උපාධි පුහුණුවක් ලබා ඇති නමුත්, අදාළ විශේෂඥතාව අනුව නියමිත විදේශීය පුහුණුව සම්පූර්ණ කිරීම Board Certification සඳහා අවශ්ය ක්රියාවලියේ කොටසකි. එවැනි වෛද්යවරයෙකු රෝහලක එකම විශේෂඥ මට්ටමේ වෛද්යවරයා ලෙස යොදවා ස්වාධීනව විශේෂඥ තීරණ ගැනීම, ක්රියා පටිපාටි සිදු කිරීම, අනෙකුත් වෛද්යවරුන් අධීක්ෂණය කිරීම සහ හදිසි විශේෂඥ සේවා සැපයීම අපේක්ෂා කරන්නේ නම්, එම වගකීම් සඳහා ඔහුට හෝ ඇයට ඇති නිල බලතල සහ වෘත්තීය හා නීතිමය ආරක්ෂාව පැහැදිලිව තහවුරු කළ යුතුය. විශේෂඥ මට්ටමේ සේවාවක් අපේක්ෂා කරමින් පත්වීම් ලිපියේ වෛද්යවරයාගේ තනතුර සහ ඔහුගෙන් අපේක්ෂා කරන සැබෑ වගකීම අතර පරතරයක් පැවතීම බරපතළ ගැටලුවකි. එබැවින් සෞඛ්ය අමාත්යාංශය විසින් මෙතෙක් කාලයක් සිදුකරන ලද පරිදි මෙම වෛද්යවරුන් වැඩබලන විශේෂඥ වෛද්යවරුන් ලෙස පත්වීම ලබාදීම සිදු කළ යුතුය
දුෂ්කර රෝහල්වල සේවා තත්ත්වයන් පිළිබඳව ද අවධානය යොමු කළ යුතුයි.
විශේෂඥ වෛද්ය සේවාව දිවයින පුරා පවත්වාගෙන යාම අත්යවශ්යය. එහෙත් ඇතැම් ද්විතීයික මට්ටමේ රෝහල්වල යම් විශේෂඥතාවකට සිටින්නේ එක් වෛද්යවරයෙකු පමණි. එවැනි අවස්ථාවල හදිසි රෝගී තත්ත්ව, ශල්ය තීරණ සහ හදිසි උපදේශන සඳහා එම වෛද්යවරයාගේ සේවය ඕනෑම වේලාවක අවශ්ය විය හැකිය. එබැවින් ප්රායෝගිකව පැය 24, දින 7, වසරේ දින 365ක අඛණ්ඩ වගකීමක් විශේෂඥ වෛද්යවරුන් විසින් දරනු ලැබේ. නමුත් එවැනි වගකීම් පවරන ස්ථානවල ප්රමාණවත් හා ගුණාත්මක නිල නිවාස, හදිසි ප්රවාහන පහසුකම් සහ පවුල සමඟ පදිංචි වීමට අවශ්ය පහසුකම් පවා බොහෝ විට ප්රමාණවත් නොවේ. එබැවින් දුෂ්කර ප්රදේශවල විශේෂඥ සේවාව පවත්වාගෙන යාම සඳහා නිල නිවාස, ප්රවාහන පහසුකම්, සාධාරණ ඇමතුම් රාජකාරි (On-call) ක්රමවේද සහ අවශ්ය සහායක මානව සම්පත් සැපයීම රජයේ වගකීමකි.
විශේෂඥ වෛද්ය සේවයේ වටිනාකමට සාධාරණ ප්රතිලාභයක් අවශ්යයි.
විශේෂඥ වෛද්යවරයෙකු බිහිකිරීම සඳහා වසර ගණනාවක වෛද්ය අධ්යාපනය, පශ්චාත් උපාධි පුහුණුව, විභාග, පුහුණු කාලය සහ අනිවාර්ය විදේශීය පුහුණුව ඇතුළත් දීර්ඝ ක්රියාවලියක් අවශ්ය වේ. එමෙන්ම විශේෂඥ වෛද්යවරයෙකුට රෝගී ප්රතිකාරයට අමතරව හදිසි සේවා, කනිෂ්ඨ වෛද්යවරුන් පුහුණු කිරීම, පශ්චාත් උපාධි අධ්යාපනය, පර්යේෂණ, සායනික විගණන සහ ජාතික සෞඛ්ය ප්රතිපත්ති සඳහා දායක වීම වැනි විශාල වගකීම් පැවරේ. එවැනි වගකීම් සහ පුහුණු මට්ටමට සාපේක්ෂව ශ්රී ලංකාවේ රාජ්ය අංශයේ විශේෂඥ වෛද්යවරුන්ගේ වැටුප් හා ප්රතිලාභ ප්රමාණවත් නොවන බව රජයේ වෛද්ය නිලධාරීන්ගේ සංගමය අවධාරණය කරනු ලැබේ. ලෝකයේ මෙන්ම දකුණු ආසියානු කලාපය තුළ ද වෛද්යවරුන්ට සහ විශේෂඥ වෛද්යවරුන්ට අවම වැටුප් ගෙවන රටක් ලෙස ශ්රී ලංකාව හැඳින්විය හැක. විශේෂඥ වෛද්ය සේවාවේ වටිනාකම නිසි ලෙස හඳුනා නොගැනීම, දුෂ්කර සේවා ස්ථාන වලට වෛද්යවරුන් රඳවා ගැනීම සහ රට තුළ විශේෂඥ මානව සම්පත රැකගැනීම සම්බන්ධයෙන් ද දිගුකාලීන ගැටලු ඇති කළ හැකිය.
ඉගැන්වීම් සහ පර්යේෂණ ද සෞඛ්ය සේවාවේ කොටසකි.
විශේෂඥ වෛද්යවරුන් රෝගී ප්රතිකාරයට පමණක් සීමා නොවේ. ඔවුන් අනාගත වෛද්යවරුන් පුහුණු කිරීම, කනිෂ්ඨ වෛද්යවරුන්ට මඟපෙන්වීම, පර්යේෂණ හා සායනික විගණන සිදු කිරීම සහ වෛද්ය සේවාවේ ගුණාත්මකභාවය ඉහළ නැංවීම සඳහා මූලික දායකත්වයක් ලබාදේ. එබැවින් වැඩිවන සායනික සේවා බර සමඟ ඉගැන්වීම්, පර්යේෂණ, අඛණ්ඩ වෘත්තීය සංවර්ධනය සහ වැඩිදුර අධ්යයන සඳහා අවශ්ය කාලය, ආයතනික සහාය සහ ඒ සදහා සුවිශේෂි දිරිගැන්වීම්ද ලබාදිය යුතුය.
බොහෝ විට දුෂ්කර සහ දුරස්ථ ප්රදේශ වලදි එකම විශේෂඥ වෛද්යවරයා රෝහල් කිහිපයක රාජකාරි ආවරණය කරනු ලබන අතර එසේ සපයන අමතර සේවාව සෞඛ්ය අමාත්යංශය මගින් කිසිදු ඇගයිමකට ලක් නොවේ. එමෙන්ම එම රාජකාරි ආවරණය දුරස්ථ මට්ටමෙන් සිදු කරන අවස්ථාවකදි ඒ වෙනුවෙන් පැහැදිලි නීතිමය ආවරණයක් සපයන ක්රමවේදයක්ද ස්ථාපිත වී නොමැත.
රජයේ වෛද්ය නිලධාරීන්ගේ සංගමයේ ඉල්ලීම.
රජයේ වෛද්ය නිලධාරීන්ගේ සංගමයේ අරමුණ දුෂ්කර ප්රදේශවල විශේෂඥ සේවා නතර කිරීම නොවේ. අපගේ අරමුණ වන්නේ රටේ සෑම ප්රදේශයකම ජනතාවට ආරක්ෂිත, ගුණාත්මක සහ තිරසාර විශේෂඥ සේවාවක් ලබාදීමයි.
ඒ සඳහා සෞඛ්ය අමාත්යාංශය විසින්,
• පවරන වගකීම්වලට ගැළපෙන නිසි පත්වීම් සහ තනතුරු නාම ලබාදීම
• විශේෂඥ මට්ටමේ වගකීම් සඳහා පැහැදිලි වෘත්තීය හා නීතිමය ආරක්ෂාව තහවුරු කිරීම
• දුෂ්කර සේවා ස්ථාන සඳහා නිල නිවාස, ප්රවාහන සහ අනෙකුත් මූලික පහසුකම් ලබාදීම
• සාධාරණ රාජකාරි හා On-call ක්රමවේදයක් ඇති කිරීම
• විශේෂඥ වෛද්යවරුන්ගේ වැටුප් සහ දීමනා ඔවුන්ගේ පුහුණුව, වගකීම් සහ සේවා බරට සාපේක්ෂව නැවත සමාලෝචනය කිරීම
• ඉගැන්වීම්, පර්යේෂණ සහ වෘත්තීය සංවර්ධනය සඳහා අවශ්ය පහසුකම් සැපයීම වහාම සිදු කළ යුතුය.
රටේ නිදහස් සෞඛ්ය සේවය ශක්තිමත් කිරීමට නම් එහි විශේෂඥ වෛද්ය මානව සම්පත ආරක්ෂා කිරීම අත්යවශ්ය වේ.
එමගින් රටේ සියලු ප්රදේශවල ජනතාවට විශේෂඥ වෛද්යවරුන්ගේ සේවාව ලබා ගැනීමට ඇති අයිතිය තහවුරු කරන අතරම විශේෂඥ වෛද්යවරුන් ඇතුළු වෛද්යවරුන් රට තුළ රඳවා තබා ගැනීමට ද ඔවුන්ගේ සේවාව වඩාත් යහපත්ව ජනතාව වෙත ලබා ගැනීමට ගැනීමට ද කටයුතු කළ හැකි බව අප අවධාරණය කර සිටිමු.
Government Medical Officers' Association Calls for Enhanced Support for Specialist Healthcare Services
The Government Medical Officers' Association (GMOA) has issued a striking media statement emphasizing the necessity for proper authority, legal protections, and fair working conditions in maintaining specialist medical services across the country.
Focus has been drawn to the Health Ministry's recent appointments that designated doctors awaiting overseas training as medical officers, highlighting an urgent need for adequate placements considering the demanding responsibilities they are expected to undertake.
The GMOA recognizes the critical importance of delivering specialist healthcare to people in remote and underserved regions. Yet, the association insists that such services must operate under proper authority and mechanisms that align with the responsibilities assigned to each medical professional. It stresses that there must be clear appointments corresponding to specialist-level duties, ensuring that doctors have the necessary legal and professional support to make independent judgments and provide emergency care.
For many doctors who have completed their MD examinations and are ready for advanced postgraduate training abroad, these certifications are paramount to achieving Board Certification. The disparity between the expected rank of the doctors and actual responsibilities outlined in their appointment letters poses a significant concern. The GMOA urges the Health Ministry to ensure that these appointments reflect the actual duties expected of these specialists.
Furthermore, the quality of services offered at hospitals in difficult areas demands greater attention. Currently, some secondary-level hospitals are staffed by only one specialist doctor. In emergency situations, this scenario can lead to a single practitioner being stretched thinly across multiple responsibilities, requiring continuous on-call support 24/7, 365 days a year. Unfortunately, adequate on-site housing, transportation provisions, and family accommodation facilities often fall short, raising serious concerns about the sustainability of specialist services in remote areas.
The association highlights the need for fair benefits that appropriately reflect the value of a specialist medical career. A long journey involving years of medical education, postgraduate training, examinations, and mandatory overseas training culminates in a physician who faces extensive responsibilities. Yet, despite these rigorous qualifications, the remuneration for specialists in Sri Lanka's public sector is inadequate compared to their counterparts regionally and globally.
Additionally, the GMOA points out that the nation is recognized for its low remuneration for both general practitioners and specialists within the South Asian context. This persistent undervaluation of specialist medical services and the challenges of retaining qualified professionals in remote areas could foster long-term issues within the healthcare sector.
The role of specialist doctors extends beyond patient treatment; they contribute significantly to training future physicians, mentoring junior practitioners, conducting research, and enhancing the overall quality of medical services. As the burden of clinical services continues to grow, there is an urgent need for adequate institutional support and incentives to ensure these indispensable contributions can be sustained.
In many areas, a single specialist may cover multiple hospitals, with the additional services provided often unrecognized by the Health Ministry. There is also a lack of clear legal protections to support these practitioners in distant locations, making their positions all the more precarious.
The GMOA's core objective is not to halt the provision of specialist services in challenging regions but to ensure that all citizens receive secure, high-quality, and sustainable healthcare. Therefore, it calls upon the Health Ministry to:
- Provide appropriate appointments that reflect the responsibilities assigned.
- Ensure clear professional and legal protections for specialist-level duties.
- Facilitate housing, transportation, and other essential amenities for service in difficult areas.
- Establish fair call and on-call provisions that respect the heavy workload.
- Reassess specialist salaries and benefits in relation to their training, responsibilities, and service burdens.
- Equip teaching, research, and professional development facilities urgently.
To strengthen the country's free healthcare system, securing the specialist workforce is vital. This ensures that all citizens have the right to access specialist medical care while retaining these professionals within the country, ultimately enhancing service delivery to the community.
